The rapid proliferation of Artificial Intelligence (AI) promises transformative capabilities, yet a critical oversight is emerging within organizations regarding crisis management for these sophisticated systems. New research from ISACA highlights a significant blind spot: a majority of surveyed businesses lack clarity on how swiftly they can halt an AI system during an emergency or even pinpoint the root cause of a malfunction. This inability to effectively control and diagnose AI failures presents a growing risk of unchecked, potentially irreversible damage.
According to ISACA’s report, a substantial 59% of digital trust professionals confessed to not understanding their organization’s ability to interrupt and cease an AI system during a security incident. Only a meager 21% reported confidence in intervening within a half-hour timeframe. This data paints a concerning picture of AI systems operating without adequate emergency protocols, leaving businesses vulnerable.
Ali Sarrafi, CEO & Founder of Kovant, an autonomous enterprise platform, commented on the findings, stating, “ISACA’s findings point to a major structural issue in the way that organizations are deploying AI. Systems are being embedded into critical workflows without the governance layer needed to supervise and audit their actions. If a business cannot quickly halt an AI system, explain its behavior, or even identify who is to be held accountable, the business is not in control of that system.”
**Navigating the Minefield of AI Failures and Risks**
The implications of this governance gap are profound. Only 42% of respondents expressed confidence in their organization’s ability to analyze and clarify serious AI incidents, directly contributing to potential operational failures and security risks. Furthermore, the inability to provide clear explanations to regulators and leadership following such incidents could lead to severe legal penalties and significant reputational damage.
Effective incident analysis is paramount for learning and preventing recurrence. The absence of a clear understanding of AI failures exacerbates the likelihood of repeated issues. ISACA’s findings strongly suggest that while responsible AI management and robust governance are crucial, they are frequently absent in practice.
Accountability remains a particularly nebulous area, with 20% of respondents admitting they would not know who to hold responsible if an AI system caused damage. Only 38% identified the Board or an Executive as ultimately accountable.
Sarrafi emphasized that the solution is not to decelerate AI adoption, but rather to fundamentally rethink its management. “AI systems need to sit in a structured management layer that treats them as digital employees, with clear ownership, defined escalation paths, and the ability to be paused or overridden instantly when risk thresholds are crossed. The way, agents stop being mysterious bots and become systems you can inspect and trust. As AI becomes more deeply embedded in core business functions, governance cannot be an afterthought. It has to be built into the architecture from day one, with visibility and control designed in at every level. The organizations that get this right will not reduce risk, they will be the ones that can confidently scale AI in the business.”
While the outlook might seem dire, there are glimmers of proactive engagement. Forty percent of respondents indicated that humans approve almost all AI actions before deployment, with an additional 26% evaluating AI outcomes. However, without a more robust governance infrastructure, human oversight alone may prove insufficient to identify and rectify escalating issues before they cause significant harm.
ISACA’s data underscores a pervasive structural challenge in AI deployment across various sectors. The fact that over a third of organizations do not require employees to disclose where and when AI is used in work products significantly increases the potential for blind spots.
Despite increasingly stringent regulations that place greater accountability on senior leadership, many organizations are failing to implement and utilize AI safely and effectively. It appears that many businesses are misclassifying AI risk as a purely technical problem, rather than recognizing it as a multifaceted organizational management challenge requiring careful oversight.
A fundamental shift in how AI integration and actions are handled is imperative. Without adequate governance and clear lines of accountability, businesses are effectively relinquishing control over their AI systems. In the absence of control, even minor errors can trigger reputational and financial damage from which many businesses may struggle to recover.
